1. Fill out the form on this page
2. Call HME at 800.848.4468, select option 1, 2, 3 to get a RMA number (Return Material Authorization). It is important to put the RMA number on the return label and on the box.
3. Place your equipment in a box (including packaging around your equipment for protection during shipment)

4. Place the return label on box.
5. Drop the box off at any authorized UPS service location or give it to your UPS driver.
